1. UOB Commercial Card (Corporate Member) Application Form
2. Terms & Conditions of UOB Commercial Card facility
3. UOB AutoBill Pay Form and attach a copy of your business utility bills that has been selected.
4. Joint & Several Guarantee (to be completed by Business Owner/Directors) + Photocopy of the NRIC of the Signatories.
5. Board Resolution (for companies only)

Please submit the above 5 items with the following documents :

1. Six (6) month Bank Statement,
2. Two (2) Years audited Financial Statement.

Plus the following documents
